How do I create a survey template?

You can create a new survey template by navigating to Surveys → Designer in the left-hand menu. From there, click "Create your own template."

We recommend starting with one of Woba’s ready-made templates, as many relevant questions are already included. Woba offers templates for Workplace Assessment (APV), Engagement, Onboarding/Offboarding, 1:1 Development Reviews (MUS), and Leadership Evaluation.

How do I create a question?

You can add new questions to any survey template – whether it’s an existing Woba template or one you’ve created from scratch.

If you want to create a question from the ground up, here’s how:

  1. Go to the Survey Designer and open the survey you want to add a question to.

  2. Click "Create new question."

  3. Choose a scale for how the question should be rated.

  4. Select a theme and sub-theme the question belongs to.

  5. Add a title for the question.

  6. Write the question the participant should respond to.

  7. Optionally, add help text to provide context or clarification.

  8. Click "Create."

  1. Navigate to the Survey Designer and open the survey you want to edit.

  2. Click "Add question to theme."

  3. Select a scale for how the question should be rated.

  4. Choose a sub-theme the question belongs to.

  5. Add a title for the question.

  7. Optionally, add help text to provide clarification or context.

  8. Click "Create."

 

What is a locked question?

A locked question is a question in a survey where neither the wording nor the rating scale can be edited.
This means the question is predefined by Woba and cannot be customized locally – unless you choose to deactivate or delete it (more on that below).

🛠️ Can I remove a locked question?

Yes. You have full control over which locked questions are included in your survey:

  • You can deactivate a locked question if it’s not relevant right now.

  • You can also delete a locked question entirely from a specific survey – for example, if it doesn’t align with your current focus.

How can I edit a question?

If you've created your own question – and it's not a locked Woba question – you’re free to edit it.
Please note: Questions can only be edited if the survey template is not active or completed.

Here’s how to do it:

  1. Navigate to the Survey Designer and select the relevant survey template.

  2. Click the ⋮ (three dots) next to the question you want to edit.

  3. Make your changes to the question content.

  4. Click "Save."

How do I add multiple languages?

Woba supports 15 languages in addition to Danish, and all questions are pre-translated when you use a Woba template.
You can add languages both when creating a survey template and after it has been created.

How do I delete or disable a question?

  • Go to Survey Design and open your survey

  • Locate the question you want to remove

  • Click the three dots “…” next to the question

  • Choose Deactivate to hide it (you can reactivate it later)

  • Choose Delete to remove it permanently

This is only possible if the survey hasn’t been sent

How do I make a question mandatory?

When designing a survey, you can mark specific questions as mandatory, ensuring that employees must answer them before submitting.

  • Open the survey template you want to edit

  • Create or locate the question you want to make mandatory

  • Click the icon next to the question

  • Select Settings

  • Enable the Mandatory question toggle

  • Click Save

How do I make a question conditional?

When designing a survey, you can add conditional questions – questions that only appear based on one or more specific answer options to a previous question.

Here’s how to set it up:

  1. Navigate to the template you want to edit.

  2. Create the question that will act as the trigger (the parent question).

  3. Create the question(s) you want to make conditional.

  4. Click the  next to the question you want to set as conditional
    (note: this question must come after the parent question).

  5. Select Settings.

  6. Enable Conditional question.

  7. Choose the parent question that should trigger this one.

  8. Select the answer option(s) that should activate the conditional question.

  9. Click Save.

Bemærk: A question can only be made conditional if it appears after the question it depends on. You can create multiple conditional questions based on the same parent question.

Where can I set the anonymity threshold for a questionnaire?

The anonymity threshold ensures no individuals can be identified in survey results. It’s a vital feature to build psychological safety and honesty in your organization.

 How to set the anonymity threshold

  • Go to Survey Design

  • Click on Settings in the top menu

  • Navigate to the field Anonymity Threshold

  • Select the minimum number of responses required to display data
  •  The minimum setting is 3

  • Click Save

Can I export my survey template?

In Woba, you can export a survey template as a CSV file, which you can then open and work with in a spreadsheet tool (e.g. Excel, Google Sheets or Numbers).

1. Go to Designer.

2. Select the template you want to export.

3. Click .

4. The questionnaire is now being downloaded to your computer as a CSV file.

How do I make a copy of my questionnaire?

You can copy the template to a survey if you want to reuse or modify it without changing the original one.

You can copy a survey by going to the Survey Designer: Surveys > Designer

  • Navigate to the Survey Designer.

  • Select Copy next to the survey you want to duplicate

  • An identical copy of the template will now be created

How do I test a survey?

Before sending a survey to all your employees, you can perform a final check by sending the survey only to the employees who will help test it (or just to yourself).

  • You will be directed to the survey calendar
  • Choose a start and end date
  • It’s a good idea to set the start date to today

What settings can I configure in a survey template?

Once you've designed a survey template, you can adjust its settings – as long as the survey has not yet been activated or completed.
If a template has already been used in a live survey, it will be locked and no longer editable.

You can configure the following settings:

  • Anonymity threshold – Set the minimum number of responses required before results can be viewed for a department

  • Allow response reset – Let participants delete their answers and restart the survey from scratch

  • Show results to participants – Allow participants to see their own results after completing the survey, including personalized recommendations for low-scoring themes

  • Enable chat – Allow admins to start a conversation with participants who have left comments. The participant remains anonymous but can see which admin they're chatting with

You can edit the template settings by going to:
Surveys → Designer → select survey template

  1. Navigate to the Survey Designer and select the template you want to edit

  2. Click Settings in the top menu

  3. From here, you can adjust various settings for the template

How do I create a 1:1 (MUS) survey?

You can create a 1:1 (MUS) survey in Woba the same way you would create any other survey.
This is done in the Survey Designer using either Woba’s predefined MUS template or by creating your own from scratch.

Note: If you create your own template, make sure to set the anonymity threshold to 0 to view individual-level results. 

  • Create the survey template you want to use – or select Woba’s predefined MUS template

  • Schedule the survey by going to the Calendar

  • Select the individual employees who should receive the survey. You can expand departments under the "Select recipients and employees" dropdown

  • Once an employee has submitted their response, you can view the results in the Overview and Risk Map. You can also filter by individual employees to access personal reports and insights
